You will need:

Images of you choice, I used the gorgeous artwork of Zindy S.D. Nielson, the main image is available from:My PSP Tubes, the bottom 3 are the free images from her site www.zindy.zone.dk (for the bottom tag I used the artwork of Anthony Guerra available from CILM

Supplies Zip (Font, Brushes, My Template and Twinks) HERE

Let's get started!


Open up a new image 500x500, floodfill with white, rename the layer 'background',
Copy and paste My Template as a new layer on top of the blank white canvas,
Copy and paste your tube or image as a new layer (I find that an image works best with this ;o)

Drag the tube/image layer below your template layer and resize it so that it fits (remember to make sure that resize all layers is unchecked ;o) rename this layer 'main image'
here's what mine looks like:-
 

With your image layer active select your lasso tool, point to point, feather 0, and trace around the outside of the image
 

Selections>Invert and hit delete on your keyboard, Selections>Select None.
You can add a faint drop shadow to your image if you want I added V:0 H:0 Opacity:88 Blur:6.00
Now you need 3 more images (or you could use the same, resized), copy and paste your first as a new layer (make sure it's below the template layer) and resize and position it in one of the small rectangles
select your lasso tool again and as before trace around the outside of the first small rectangle.

Selections>Invert and then again hit delete. Selections>Select None
Repeat the steps above with the other images into the other rectangles:~


Close off the visibilty by clicking the eye or glasses on the layer palette, of the background, template and main image layers which leaves all 3 small images, click on one of the small image layers on the layer palette to activate it and then Layers>Merge Visible and
lower the opacity of the new merged layer to 70 or so:~

 
Unhide all the other layers again.
Click on your main image to make it active and go to your colour palette and set your foreground to a colour of your choice, I used: #808080, and turn the background off.
Select your pen/line tool, line width 2.00, anti-alias and create as vector both checked, line style: dot, and draw around your main image (tip! Holding down the shift key on your keyboard gives you nice straight lines ;o). when your happy with it, convert to raster layer
rename this layer 'line'.

Add new raster layer name it 'brushes' and drag it above your background layer
have fun with adding some brushes of your choice, I have added some brushes in with the supplies if you want to use them ;o)
(if you get in a mess like I do lol! then you can add another raster layer to apply more brushes so that you can move them where you like and rotate, flip and mirror them without moving the others that you've just placed!)
I lowered the opacity of one of the brush layers.

Add the artists copyright and your watermark etc as a new layer on the top of your layer palette
(You can merge all the layers visible now if you're like me and want to save as a blank to add names on for people ;o)
Add the text for your name as a new layer, I used:- Foreground: #5A5A5A Background: #BFBBC3 Font: Dali and added an inner bevel:

Bevel: 2
Width: 15
Smoothness: 21
Depth: 7
Ambience: 0
Shininess: 40
Colour: White
Angle: 49
Intensity: 34
Elevation: 55

And drop shadow:
V:-2 H:2
Opacity: 55
Blur: 5.00

Place your name text to your liking and then merge visible, save as jpeg or continue to the next steps to add some twinks!

To add the twinks, copy and paste your tag into animation shop and open the twinks, you'll see that the twinks have 16 frames,
go back to your tag and duplicate it so that it has 16 frames, (it will go to 17, so click on the last frame and hit delete ;o)
Click on the twinks animation and Edit>select all and Edit>copy (make sure the slider is at frame 1 ;o)
back to your duplicated tag and also make sure the slider is at frame 1 and Edit>Select all Edit> Paste into selected frame and place it where you like it.
If you want to add more twinks the go back to the twink animation and click 'undo' and repeat the steps.
Play your animation to check it plays ok and then crop your tag and save as gif :o)
